Today’s medical professionals are able to dictate into a computer, phone or digital recorder in no detailed fashion (e.g., in the normal course of speaking), have the speech engine recognize what the clinician wants and/or means, and then apply the commands and/or structured words, respectively. For example, by dictating “Cardiology Report for Carol Carter” (i.e., what the clinician wants), the speech engine knows to show the Cardiology report template occupied with Carol Carter’s name and other essential, demographic data. By dictating the cardiology report narrative (i.e., what the clinician means), the computer recognizes the narrative context and intent and condenses a complete, correct, and structured data-based document “on the fly." This allows medical professionals to document patient clinical information more efficiently and clearly. The next generation of medical speech recognition software routinely embeds document structures, such as sections, paragraphs, lists, as well as specific concepts. Modifiers and relationships all of which can be encoded using standard medical nomenclatures, such as SNOMED, and standard medical classifications, as well as ICD 10. Also, this allows clinicians to keep their documentation actions and desired workflows yet still create organized and encoded documents for EHRs and reimbursement.
The structured, clinical narrative reports/notes can be designed in several ways. Voice files dictated through digital dictation systems can be sent through a front or back-end speech recognition engine with speech recognition (using NLP) and revised by either the clinician or a transcriptionist. Voice files dictated through digital dictation systems can be transcribed by transcriptionists using text/transcription systems with documents containing pre-defined XML fields. In either case, exact data fields within the notes/reports, such as lab values and quality measure information can be marked, extracted and electronically embedded/exported into the applicable EMR data fields within EMR systems.
